Grievance Form
All Grievance Forms are first opened by our Office Manager and then given to the appropriate supervisor. If your concern involves the Executive Director, the form will be given to the EDI Board Chair. The appropriate staff member will contact you regarding your concern within 5 business days. Assistance to complete this form will be provided if needed.
